How Case Managers Coordinate Recovery Support in Brooklyn Rehab Centers

Published on

Treatment breaks down in the gaps between stages far more often than it breaks down inside them. A patient finishes detox and waits two weeks for a rehab bed. A prescription lapses between discharge and a first outpatient appointment.

Care coordination exists to close those gaps. Under one roof in Coney Island, a clinically led addiction treatment center has a single team handle detox, rehab and discharge planning. Discharge planning begins on day one rather than at the end, which is the part that makes coordination possible rather than theoretical.

‍

What coordination covers across a treatment stay

Coordination is the work of making separate pieces of care behave like one plan. A stay includes a medical evaluation, a psychiatric evaluation, medication management, counseling and family sessions. Somebody has to hold that whole picture. Otherwise each piece runs correctly in isolation and the patient experiences a series of disconnected appointments.

The work is mostly unglamorous. It means the counselor knows what the psychiatric evaluation found before the first session rather than after the fourth. It means family counseling is scheduled when the relatives can actually attend. It means an insurance authorisation is renewed before it lapses, not after a patient has been told their coverage ended. Our clinical team works under Program Director Vincent Marchese, who holds an MSW from Fordham University and an Advanced CASAC. Executive Directors Ami Mitteldorf and Eric Moore lead the organisation, and Mitteldorf has more than 25 years in addiction treatment.

‍

Why a single location removes most coordination failures

Fragmented care is the norm in addiction treatment across much of New York. Detox happens at one facility, rehabilitation at another, medication management somewhere else again. Each handoff is a point where a patient can be lost. Waits between stages are common, and every provider has levels of care explained differently. A person told to wait ten days for a bed is being asked to hold a fragile position alone.

‍

Keeping detox and residential rehabilitation under one roof removes those handoffs entirely. The patient does not repeat their history to a new intake worker. The clinical team that assessed them on arrival is the same team planning their discharge, so nothing has to be reconstructed from paperwork. Medical Director Dr. Ramsey Joudeh and Assistant Medical Director Joseph Turkel oversee medical continuity across both stages. Nursing staff are on the unit around the clock during detox.

‍

What gets coordinated before a patient is admitted

Coordination starts before admission rather than at it. The intake conversation covers insurance verification, medical review and level of care, and each of those shapes what happens next.

  • Insurance verification. We confirm coverage at no cost before admission, across all major Medicaid plans including Healthfirst, MetroPlus, Fidelis Care, Molina and Wellcare, plus Medicare and self-payment.
  • Medical review. Substance history, quantity and duration determine whether detox is needed before rehab.
  • Level of care. The clinical picture decides what is appropriate rather than what is available.
  • Referral coordination. Where a doctor, hospital or case worker has made the referral, their information informs the plan.
  • Admission logistics. What to bring, when to arrive, and what the first day involves.

That pre-admission work is why the first 24 hours can move quickly. A patient arrives and moves into the intake evaluation and a psychiatric assessment inside that window. The opening day is not spent on paperwork that could have been handled earlier.

Our intake team will walk you through the first 24 hours, explain how detox, rehab, and discharge planning connect, and verify your insurance for free. Nothing about the call commits you to admission.

‍

How coordination carries treatment into discharge

Discharge planning begins on day one, and the stay averages 21 to 28 days. That gives the clinical team weeks to build something that holds. A plan assembled in the final 48 hours is a list of phone numbers. A plan built over three weeks has appointments confirmed, prescriptions arranged and the patient's actual circumstances accounted for.

That plan can include continued counseling referrals, relapse prevention strategies and connections to 12-step support through AA and NA. Medication-assisted treatment continues where the medical team determines it applies. For patients leaving on MAT after opioid detox, continuity is a clinical question. A gap in medication between discharge and the first outpatient appointment is a recognised relapse risk. Arranging that handoff while the patient is still in the building is straightforward. Arranging it afterwards frequently is not.

‍

What families and referring clinicians should expect

Families often want a single point of contact and a clear account of what is happening, and both are reasonable requests with one limit. HIPAA governs what can be shared and with whom, and the patient authorises that in writing. Family counseling is part of the residential program, within whatever the patient has authorised. The level of involvement is set with the patient and the clinical team.

Referring clinicians, social workers and case workers generally want to know that the referral landed and what the plan is. The same authorisation rules apply, and once they are in place the coordination runs through the clinical team rather than through a general line. We are OASAS-licensed by New York State, hold CARF Three-Year Accreditation on both our detox and residential programs, and are LegitScript certified.

Recovery works best when nobody has to piece the plan together alone. Call (646) 347-1893 or submit the form at surfpointrecovery.com to talk with our intake team, whether you’re a patient, a family member, or a referring clinician. We’ll verify your coverage at no cost and explain how care is coordinated from day one through discharge, with no obligation.
